The following describes how Simona DeMarchi Paediatric Therapy (“SDPT,” “Simona DeMarchi”, “I,” “me”, “my”, “we” or “us”) uses and disseminates information you provide through www.simonademarchi.com and the SDPT pages and accounts on Instagram® Facebook®, and LinkedIn® (“Sites”). If you ever have questions about this Privacy Policy, please contact SDPT directly. Please also review the SDPT Terms and Conditions of Use, which also govern your use of the sites, as well as any services and/or products you may purchase from SDPT. 

By accessing or using the Sites and/or purchasing my services and/or products, you agree to accept both this Privacy Policy and to abide by the Terms and Conditions of Use, including the terms and conditions of this Privacy Policy that relate to the ways that your personal information may be collected, used, and/or disclosed. 

You can withdraw your consent at any time by contacting SDPT directly. Please note that if you choose to withdraw your consent, I may not be able to provide you with my services, products, or communications.

APPLICABLE LEGISLATION

At SDPT, I abide by the rules and regulations imposed by the College of Physiotherapists of Ontario, the licensing body of which I am a member in good standing, with respect to the collection, use, and disclosure of your personal health information. 

COLLECTION OF YOUR PERSONAL INFORMATION

Unless expressly noted otherwise, we do not collect personally identifiable information from users of my Sites. When you visit my Sites, some information about your computer hardware and software, such as your IP address, domain name, browser type, access time and referring website addresses, is inherently and automatically collected. An example of when this information may be used is in implementing improvements and analyzing the Sites and for troubleshooting purposes. As the SDPT sites are public, you may visit the Sites without telling us who you are. However, in order to gain access to certain parts of the Sites or particular content, or to purchase particular services and/or products, you may be required to provide us with personal information. We require only the information that is reasonably required to enter into a service relationship with you.

FOR PARENTS OR LEGAL GUARDIANS

Any child visiting SDPT for an initial physiotherapy appointment is required to be accompanied by at least one parent or legal guardian. During the initial intake, the child’s parent or legal guardian will be asked to confirm that they authorize SDPT to collect, use, and disclose their and their child’s personal information as applicable and in connection with the services that are provided. For subsequent visits to SDPT, the child may be accompanied by any responsible adult who has the child’s parent(s) or cargiver(s) consent to do so. 

SDPT does not seek to collect, use, or disclose a child’s personal information without the consent of their parent or legal guardian and we limit the use of a child’s personal information for and in connection with the services provided to the child.

COLLECTION OF PERSONAL INFORMATION

We collect and maintain different types of personal information which may include, but is not limited to, the following:

  • contact and identification information, such as your name, email address, telephone number, and home address. For example, you will be asked to provide this information when you book an appointment and/or purchase a product or service from SDPT. 

  • personal health information which is relevant to your use of our services. 

  • payment information, including your credit card number and billing address.

Please note that we also collect personal information about your child when you request that we provide our services for the benefit of your child. We do not collect a child’s personal information without the consent of the child’s parent or legal guardian. In the case of a minor, if you are not the parent or legal guardian of the minor, you represent that you have received the minor’s parent’s or legal guardian’s permission to share the minor’s information with SDPT. 

INFORMATION FROM THIRD PARTIES AND TRACKING TECHNOLOGIES

The SDPT website uses cookies (“Cookies”). A Cookie is a data text file sent from a website to your browser, for the purpose of identifying the user and allows access to portions of the website. Cookies may be stored within your system. This allows us to distinguish you from other users of the website and helps us to provide you with a good experience when you use the website. Cookies also allow us to improve our website and the services provided at SDPT. 

USE OF PERSONAL INFORMATION

We collect and use personal information to:

  • establish and manage our service relationship with you;

  • allow you to participate in, subscribe to, and receive information, news, updates, and promotional materials from SDPT and, as applicable, from third parties;

  • evaluate the ways in which our services are used and how to improve our services to better meet the needs of our clientele.

Please keep in mind that if you disclose personally identifiable information in a public manner through the Sites, this information may be collected and used by others accessing those portions of the Sites. We do not accept any liability associated with your voluntary disclosure of the same.

As your use of the Sites may pertain to third parties, you are responsible for reviewing the privacy statements and policies of other websites you choose to link to or from the Sites, so that you may understand how those sites collect, use and store your information. SDPT is not responsible for the privacy statements, policies, content, of any other websites.

By consenting to our collection, use, and disclosure of your personal information through the SDPT website and in connection with the services we provide, you allow us to fulfill the purposes for which we collect and use your personal information and the personal information of any minor that you provide.

POLICY CHANGES

This Privacy Policy may be updated from time to time. Any modifications will be dated and effective immediately from the date posted. Steps will be taken to notify users of any modifications, however, you are encouraged to check this Privacy Policy for updates on a regular basis. When the Privacy Policy has been updated, the date that the revisions were mad will be noted at the top of the Policy. Your continued use of a site following any changes means you accept and agree to any changes.
